Nature Med | CT带来的辐射积累可能增加儿童/青少年白血病风险

CT是一种非常重要的医学检测手段,可人们很早就知道CT带来的辐射可能增加患癌风险。医学界也在降低儿童接受CT检查的次数和辐射剂量,比如尽可能用MRI、超声替代,不过有时对儿童进行CT检查仍是必须。
目前公认中等剂量(≥100mgy)到高剂量(≥1gy)的电离辐射暴露是儿童和成人患白血病的危险因素。然而,儿童和青少年通常接受的诊断性CT检查剂量范围(<100mgy)的低剂量辐射与白血病风险的关系目前尚不清楚。
近日,由 Elisabeth Cardis 团队发表于 Nature Medicine 题为 Risk of hematological malignancies from CT radiation exposure in children, adolescents and young adults 的文章, 对9个欧洲国家的近100万名22岁前接受过CT检查的人进行了多国队列(EPI-CT)随访,发现儿童和青少年进行CT检查后,即使在较低剂量范围内也存在患血液肿瘤的风险升高,包括淋巴瘤和骨髓瘤。在接受CT检查的每1万名儿童中(平均剂量为8mgy),预计有1-2人会在随后的12年中因辐射暴露而患上血液系统恶性肿瘤。

该研究纳入了948174名22岁以下的患者,总共1,331,896次CT检查(每人平均1.52次,标准差为1.46次)。这些患者在首次接受CT检查后接受了至少2年的随访,平均随访时间为7.8年。研究结果显示,共有790例患上了血液系统恶性肿瘤,其中包括578例淋巴细胞恶性肿瘤和203例髓系恶性肿瘤和急性白血病。

研究发现,在受到辐射剂量≥10 mGy的患者中,患有所有血液系统恶性肿瘤的相对危险度均增加。相对于接受<5 mGy辐射剂量的患者,接受≥50 mGy辐射剂量的患者的相对危险度增加了2.66倍。每增加100 mGy的辐射剂量,患血液系统恶性肿瘤的估计超额相对危险度增加了1.96倍。随着辐射剂量的增加,淋巴细胞恶性肿瘤、髓系恶性肿瘤和急性白血病的风险估计值均增加。每10万人的人群中,每增加100 mGy的辐射剂量,就有17.7例额外的血液系统恶性肿瘤发生。

每增加100 mGy的辐射剂量,患非霍奇金淋巴瘤(NHL)的相对危险度增加了2.51倍,患霍奇金淋巴瘤(HL)的相对危险度增加了1.24倍。即使在10-15mGy的辐射剂量范围内,患有非霍奇金淋巴瘤,特别是成熟B细胞肿瘤的相对患病危险度也比那些接受参考辐射剂量的人更高。在白血病方面,除慢性淋巴细胞白血病(CLL)外,患病的相对危险度也随辐射剂量增加而提升。

综上,这项大规模研究旨在直接评估儿童和青少年接受CT辐射暴露的癌症风险。结果表明,CT辐射累积剂量与所有血液系统恶性肿瘤的风险之间均呈正相关。辐射剂量每增加100 mGy,患上血液系统恶性肿瘤的风险相对增加1.96倍。在接受CT检查的每1万名儿童中(平均剂量为8mGy),预计有1-2人会在随后的12年中因辐射暴露而患上血液系统恶性肿瘤。
该研究结果进一步强化了有关低辐射剂量增加癌症风险的证据,强调了对儿科CT检查的次数合理化和剂量优化的迫切需求。

原文摘要(Abstract)

Over one million European children undergo computed tomography (CT) scans annually. Although moderate- to high-dose ionizing radiation exposure is an established risk factor for hematological malignancies, risks at CT examination dose levels remain uncertain. Here we followed up a multinational cohort (EPI-CT) of 948,174 individuals who underwent CT examinations before age 22 years in nine European countries. Radiation doses to the active bone marrow were estimated on the basis of body part scanned, patient characteristics, time period and inferred CT technical parameters. We found an association between cumulative dose and risk of all hematological malignancies, with an excess relative risk of 1.96 (95% confidence interval 1.10 to 3.12) per 100 mGy (790 cases). Similar estimates were obtained for lymphoid and myeloid malignancies. Results suggest that for every 10,000 children examined today (mean dose 8 mGy), 1-2 persons are expected to develop a hematological malignancy attributable to radiation exposure in the subsequent 12 years. Our results strengthen the body of evidence of increased cancer risk at low radiation doses and highlight the need for continued justification of pediatric CT examinations and optimization of doses.
